Thai public health officials have described nuad kasai in terms quite different from Karsai Nei Tsang: back first, then abdomen down to the groin, with no genital work.

Training reference, not a self-study course.

This page summarises how published sources describe a technique, with the anatomy and safety rules a practitioner needs. It does not replace supervised, in-person training, and it is not evidence that the technique treats any condition. In Thailand, Ministry of Public Health officials have said kasai massage is a treatment that belongs in licensed Thai traditional medicine settings, not general massage shops.

What Thai officials described

In August 2012, as advertisements for "kasai massage" multiplied, the Department of Thai Traditional and Alternative Medicine explained publicly what the practice involves. According to the head of its Thai massage working group, as reported by MGR Online:

  • the massage starts on the back, working the spine down to the sacrum and the tailbone to relax the muscles;
  • it then moves to the front, working the abdomen downwards towards the groin, following specific lines and pressure points, towards the perineum;
  • the client wears loose cotton trousers or a sarong; nudity is not part of it;
  • it does not target the male genitals, and it requires a highly skilled practitioner.

The Department's director-general said the massage was used to treat prostate enlargement and kidney problems, that it may help circulation, and that there was no research supporting claims about sexual performance. A study with Thammasat University was announced; we have not found its published results.

The 2018 position on who may provide it

In December 2018, Ministry of Public Health officials quoted by Thai PBS said kasai massage counts as a treatment. In their view it may only be provided by clinics or hospitals offering Thai traditional medicine, after an assessment, and not by general health massage shops. They also warned that off-site, unsupervised sessions carry risks of infection, injury and sexual abuse.

How the sequence compares

  • Same: abdominal work towards the groin, attention to the lower back and sacrum, a slow, skilled approach.
  • Different: the Thai description starts with the back, keeps the client clothed and excludes the genitals; Karsai Nei Tsang starts with the abdomen and includes genital techniques.

For the individual techniques used on the back and abdomen, see back, sacrum and buttocks, opening the abdomen and groin and inner thigh. The sen lines used in Thai massage are explained in Thai origins and male massage traditions.
